Insurance Information      

For your convenience, we accept most insurance plans.  Please call our office at (949) 264-6744 or your insurance provider to find out if your insurance plan is contracted with our office

If we are contracted with your insurance company, we will file claim to your insurance company as a courtesy to you. Please bring your current insurance card, personal identification (i.e.: driver’s license, state ID card), and provide us with all your insurance information.  We request any copays, deductibles, and any services not covered by your insurance plan be paid at the time the service is rendered.  Please be aware if your insurance company has not paid your claim in full within the forty-five (45) days from the date of service, any unpaid balance is ultimately your responsibility.
