Office Policy      

Thank you for choosing our practice for your endodontics needs.  The following office policies are in place so that we can provide you with the utmost personalized care in a timely fashion.  If you have any questions or concerns, please give us a call 949-264-6744

Payment

Full payment is due at the time of service.  We accept Cash, Debit and Visa, MasterCard, Discover Card, and American Express.  We also offer Care Credit which is an extended payment plan prior credit approval

If we are contracted with your insurance company, we request any co-pays, deductibles, and any services not covered by your insurance plan be paid at the time the service is provided.

​Late Arrivals

Late arrivals can cause schedule delays for patients who did  promptly at their appointment.  Therefore, if you are more than thirty (30) minutes late for your appointment we may need to reschedule you to another day to allow enough time for your treatment

Cancelation Policy

We treat one patient at a time. As a result, your appointment is especially reserved for you.  If, for any reason you cannot keep a scheduled appointment or will be delayed, please call us as soon as possible.

Minors – Patients under age of 18

If the patient is under 18 years old, he or she must be accompanied by a parent or legal guardian.  Payment of services for the treatment is the responsibility of the accompany adult
