Your First Visit      

We welcome you as a patient and appreciate the opportunity to provide you with endodontic services.  Your first visit will consist of a consultation where we will perform a few simple tests, take a few X-rays to confirm recommended treatment.  We will go over your diagnosis and treatment options.  Treatment is usually done on the same day as the consultation.  However, a complex medial history may need to schedule treatment on another day

Please assist us by providing the following information at the time of your appointment:

• Your Referral Card

• Personal Identification (Driver’s License or State ID Card)

• A list of current Medications, past Surgeries, and any known Allergies

• If you have Dental Insurance, please bring your insurance card

• If you plan on using Care Credit, please bring your card  

Important:

• A parent or guardian must accompany all patients under the age of 18

• Please alert the office if you have a medical condition that may be of concern prior to treatment or if you are required medication prior to dental procedure.
